My Health Check

The ‘My Health Check’ app is designed to support you following your NHS Health Check in Kent.

Record your blood pressure, cholesterol, weight and activity results onto the app and get your heart age! Then follow the advice of your health checker and the information in the app to track your progress to a healthier lifestyle.

You can download apps that can help you to quit smoking, lose weight or get your blood pressure down. Or set goals for yourself, like being more active or cutting back on booze.

This app has been designed by Kent Community Health NHS Foundation Trust and Tinderhouse and is intended for clients in Kent who are eligible for an NHS Health Check. If you haven’t had an NHS Health Check, you can still use the app, but you may not get a correct heart age score. Merekam tekanan darah, kolesterol, berat badan dan aktivitas hasil Anda ke aplikasi dan usia jantung Anda! Kemudian ikuti saran dari checker kesehatan Anda dan informasi dalam aplikasi untuk melacak kemajuan Anda untuk gaya hidup sehat.

Anda dapat men-download aplikasi yang dapat membantu Anda untuk berhenti merokok, menurunkan berat badan atau mendapatkan tekanan darah Anda turun. Atau menetapkan tujuan untuk diri sendiri, seperti menjadi lebih aktif atau memotong kembali pada minuman keras.

Aplikasi ini telah dirancang oleh Kent Community Health NHS Foundation Trust dan Tinderhouse dan ditujukan untuk klien di Kent yang berhak untuk NHS Kesehatan.
